Application Process
.

Two fellowship positions are available each year, beginning July 1. Applicants trained in physical medicine & rehabilitation, anesthesia, neurology, or psychiatry are encouraged to apply. Applications are accepted through the Electronic Residency Application Service.

A complete application includes the following:

• Personal statement
• Curriculum vitae
• Three letters of recommendation:
  • Residency Director
  • Pain Physician (interventional or non-interventional)
  • Applicant’s choice
• USMLE and COMLEX scores
• Board scores (if applicable)

If selected, all applicants must meet the following requirements by start of program:

• Board eligible or board certified
• Fully licensed in the State of Wisconsin
• ACLS certification

We observe all rules and conditions of the NRMP match.
