Application Process
 


Two fellowship positions are available each year, beginning July 1. The program accepts one anesthesia-trained physician and one physical medicine & rehabilitation-trained physician. Applications are accepted through the Electronic Residency Application Service.

A complete application includes the following:

  • Personal statement
  • Curriculum vitae
  • Three letters of recommendation:
    • Residency Director
    • Pain Physician (interventional or non-interventional)
    • Applicant’s choice
  • USMLE and COMLEX scores
  • Board scores (if applicable)

If selected, all applicants must meet the following requirements by start of program:

  • Board eligible or board certified
  • Fully licensed in the State of Wisconsin
  • ACLS certification

The program will begin reviewing applications in March 2024. Invitations for a personal interview will be offered in early April 2024 to promising candidates, especially those with a commitment to research.
